This test is designed to allow you to answer questions that may indicate that a diabetic condition exists. It is Not intended to replace or be a substitute for a clinical exam, which should be completed in an appropriate setting, such as your doctor's office. It is Now estimated that over 20 million American's (177 million people worldwide) have diabetes and at least half of them don't know it! Please take a moment to answer the test questions.
I go to the bathroom (urinate) often. (every 2-3 hours)
I am always thirsty and/or hungry
In spite of my appetite, I am suddenly losing a lot of weight
I am always fatigued (weak, tired) and/or drowsy
I am irritable and have mood changes
I am nauseous and/or vomit often
I have blurred vision
I have a tingling or numbness in legs, feet or fingers
I have frequent or recurring skin, gum and/or urinary tract infections
I have frequent itching of my skin and/or genitals
I have s Low healing of cuts and bruises
My family history show diabetes (5 Points)
My age is: 45 to 65 (5 Points); Over 65 (10 Points)
I am: Overweight (5 Points); Obese (over 20% Overweight) (10 Points)
For women, if you have a history of gestational diabetes or the delivery of a baby over 9 pounds (5 Points)
If you are Asian, African-American, Hispanic, Pacific-Islander or Native American medical data shows that within these ethnic groups there are extremely High diabetic populations (5 Points)
Indications of AN (Acanthosis Nigricans) a skin condition characterized by darked, velvety and/od thickened skin patches. (5 Points)
NLD, Necrobiosis Lipoidica Diabeticorum, slightly raised shiny red-brown patches on Lower legs, mostly in women (5 Points)
